چکیده

Restorative proctocolectomy with ileal reservoir has become an accepted form of treatment for ulcerative colitis and familial adenomatous polyposis. Five types of reservoirs have been developed: the S pouch, J pouch, H pouch, W pouch and K pouch. This paper reviews the differences in the clinical and functional outcomes among the various reservoir designs and their sizes. From the results of the reported series, the best clinical function is apparently that associated with a large volume and highly compliant pouch which empties spontaneously. The S pouch has a high rate (3553%) of catheterization for evacuation. On the other hand, the Wand K pouches have larger and wider reservoirs with a higher compliance than of the J pouch. However, since cases in which the K pouch as well as H pouch have been used are few, a larger number of patients will be necessary to reach a conclusion regarding the best treatment. The present conclusion is that the W pouch has the best functional outcome concerning defecation.

Outcome of restorative proctocolectomy with ileal reservoir for ulcerative colitis: comparison of distal colitis with more proximal disease.

BACKGROUND An increasing number of patients with severe or refractory ulcerative colitis involving only the rectum and sigmoid colon are being offered restorative proctocolectomy with ileal reservoir but very few data are available concerning the outcome for these patients. AIM This study was designed to compare the outcome of ileal pouch procedures for distal ulcerative colitis with procedur...

anastomosis: relation with functional outcome and inflammation

Restorative proctocolectomy with pelvic ileal reservoir is a weli accepted option for the surgical treatment of ulcerative colitis. Acute pouchitis is a common complication and resembles acute ulcerative colitis. Patients with ulcerative colitis carry Escherichia coli that adhere to epithelial cells and thus this study examined whether acute pouchitis is associated with the carriage of adhesive...

The perfect pelvic pouch--what makes the difference?

The aim of this study was to determine what factors are important for the achievement of perfect anal continence after restorative proctocolectomy. One hundred patients underwent paired studies of anorectal physiology before and one year after restorative proctocolectomy with pelvic ileal reservoir (11 S, 25 J, 64 W) with stapled ileoanal anastomosis, without mucosectomy. Fifty seven patients a...

Adenocarcinoma in Ileal Pouch after Proctocolectomy for Familial Adenomatous Polyposis: Report of A Case

Restorative proctocolectomy with ileal pouch-anal anastomosis is one of the surgical treatments of choice for patients with familial adenomatous polyposis. Although the risk of cancer developing in an ileal pouch is not yet clear, a few cases of adenocarcinoma arising in an ileal pouch have been reported. We report a case of adenocarcinoma in ileal pouch after proctocolectomy with ileal pouch-a...
